What is the Arizona Division of Developmental Disabilities (DDD)?
DDD is the state program, part of the Arizona Department of Economic Security, that funds and oversees supports for people with developmental disabilities. DDD does not usually deliver services itself. It authorizes them, and qualified providers like Bella deliver them.
Who is eligible for DDD services?
In Arizona, eligibility generally covers people with autism, cerebral palsy, epilepsy, or an intellectual disability that began before age 18 and substantially limits everyday functioning. Children under six may qualify at risk of a developmental disability. DDD makes the eligibility decision, not Bella.
How do we apply?
You apply to DDD directly through Arizona DES, with documentation such as evaluations and medical records. DDD reviews eligibility and, once approved, assigns a support coordinator who builds the service plan. We are glad to talk you through what to expect, so call us and ask.
What is a support coordinator?
Your support coordinator is your DDD contact. They write and update the planning document that authorizes services, help you choose providers, and stay involved as needs change. You will work with them alongside us.
Can we choose our own provider?
Yes. Families choose among qualified providers, and you can ask your support coordinator to send a referral to Bella specifically. You can also change providers later if the fit is not right.
What does it cost?
For services DDD has authorized, DDD pays the provider, so there is normally no bill to the family. If anything would ever fall outside that authorization, we tell you before it happens, not after.
What is the difference between DTA, TRA, PTW, and ATC?
Day Treatment & Training (DTA) is structured daytime programming for skill-building and community participation. Transportation (TRA) gets people to and from those programs and community activities. Pathways to Work (PTW) is employment readiness and job coaching. Attendant Care (ATC) is one-to-one support at home with daily living and independence.
How soon can services start?
The pace is usually set by DDD authorization rather than by us. Once the authorization is in place we move quickly, and we will keep you updated at each step rather than leaving you to wonder.
Do you have Spanish-speaking staff?
Yes. We have team members who speak Spanish and we will make sure you can talk about your family's needs in the language you are most comfortable in.
What areas do you serve?
We are based in Mesa and serve Mesa and the surrounding East Valley communities. If you are just outside that area, call and ask. We will tell you honestly whether we can serve you well.

Sending us a referral, and what happens next.
How do I send a referral?
Use the referral form on this site or call the office. The form asks only for your contact information, the service you have in mind, and when we can reach you. We will call you back to take the details.
Why does the referral form ask for so little?
On purpose. This website collects contact information only: no diagnoses, no medical history, no clinical notes. Please do not enter any of that here. We gather it with you directly and keep it in Bella's HIPAA-compliant systems, which is where it belongs.
Will I hear back?
Yes. You will get an acknowledgement, and a coordinator will follow up on where the referral stands rather than leaving it silent while intake runs.
Do you have current openings?
Capacity moves month to month and differs by service line. Call or send a referral and ask about availability. We would rather tell you plainly than have you wait on a list that is not moving.

What we look for and what the state requires.
What is a Level One Fingerprint Clearance Card, and do I need one?
It is a background clearance issued by the Arizona Department of Public Safety, and Arizona requires it for direct-care and transportation roles. If you do not have one yet, apply anyway. We will walk you through getting it.
Do I need previous experience in direct care?
No. Reliability, patience, and genuine care for people matter more to us than a résumé. We train the rest, including Article 9, CPR, and First Aid.

How do I apply?
