You place people with providers every week. This page tells you what Bella takes, how to send a referral, and what happens after you do, without you having to call to find out.

What we are authorized for

Bella is a DDD-qualified vendor in Mesa and the surrounding East Valley. We deliver four services, and can combine them for the same individual.

Sending a referral

Use the referral form or call the office. The form asks only for your contact details, the service you have in mind, and when we can reach you.

Please do not enter clinical information on the website. It collects contact details only, and the database has no field for a diagnosis, a medical history, or the individual's name. We take those details from you directly and keep them in Bella's HIPAA-compliant system.

Please don't share medical or health details here. This form is for contact information only. A coordinator will call you, and any health, diagnosis, or care information is collected in person on Bella's intake paperwork. If something is urgent, call (480) 577-4253.

Capacity and honesty about it

Availability changes month to month and differs by service. Ask us and we will tell you plainly whether we can take someone now, soon, or not at all. We would rather say no than leave a person waiting on a list that is not moving.
